A North Trafford Community Partnership Case Study
Trafford Community Collective brought together 4 charities in North Trafford for a project funded by Trafford Integrated Care Board. This project is key part of the North Trafford Neighbour Plan, to bring early help to those people at risk of health challenges. Our Chief Executive Bernadette Ashcroft chairs a steering group of VCFSE groups, Trafford Leisure and GPs.
The North Trafford Community Partnership has been formed to work together across a number of community groups and charities and GPs to make sure that EVERYONE has opportunities to manage their conditions without needing to make lots of appointments or navigate what can at times be a confusing health care system.
They want to hear from anyone who is living with a long term health condition, or if you are caring for a loved one who is living with a long term health condition in particular, Diabetes, Hypertension, COPD and anyone who needs help with their emotional and mental health wellbeing, The groups specialise in offering culturally appropriate support and you can self refer to get right support for you. Contact details are on the flyer and we will we discuss how the best way to get the best support to you and your loved ones.
This is just one of the many projects which are part of the Neighbourhood Plan working being run by the Local Care Authority. If you want to make a difference and get involved with delivery of neighbourhood plans in your area contact Adrian Smith for North and West of Trafford adrian.smith@mft.nhs.uk and Dan Shelston for South and Central Trafford daniel.sheston@mft.nhs.uk.
